Job summary

Bethesda Health Group is seeking a Social Services Coordinator to evaluate and address residents' social service needs, build relationships with residents and families, and ensure timely support. Qualifications

• Bachelor's degree in social work required.
• Knowledge of growth and development and geriatric population expertise.
• Two years experience preferred; geriatric and social work experience preferred.
• Excellent organizational and communication skills.
• Ability to perform documentation in writing and electronically.

Responsibilities

• Evaluate and assess residents' social service needs.
• Develop relationships with residents and families and ensure timely fulfillment of needs.
• Initiate Social Service charting and develop an activity program.
• Monitor residents quarterly and complete charting as required.
• Complete quarterly progress notes, social service log and participate in Resident Care Plan meetings.
