Rochdale Council is seeking an experienced Social Worker to join the Front Door & Prevention Team on a temporary basis for 12 months. You will assess adults at first contact, manage risk, and develop strengths-based support plans to support independence.

The role requires registration with Social Work England, knowledge of Care Act 2014 and Mental Capacity Act, and the ability to work in a busy duty environment with multi-agency partners.

Formación

• Qualified Social Worker registered with Social Work England.
• Experience in Adult Social Care.
• Knowledge of Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
• Experience in strengths-based assessments and risk management.
• Strong communication, decision-making and partnership-working skills.
• Ability to work independently within a busy service.

Responsabilidades

• Manage referrals and undertake timely screening and triage of new contacts.
• Complete Care Act assessments, Mental Capacity Assessments, and risk assessments.
• Work collaboratively with individuals, families, health partners and community organisations to identify preventative solutions.
• Develop strengths-based support plans that maximise independence and wellbeing.
• Provide information, advice, guidance and signposting to appropriate services.
• Manage safeguarding concerns in line with statutory duties and local procedures.
• Maintain accurate records and complete assessments within required timescales.
• Work effectively within a duty environment, prioritising urgent work and managing competing demands.
