Job summary

Rutland County Council is seeking an experienced Social Worker to join our Adult Social Care Contact and Response Team in a 2-year fixed-term role. The post sits at the front door of Adult Social Care and focuses on triage, assessment and short-term intervention to promote independence and early intervention.

You will undertake assessments, support planning and short-term casework, working closely with partner agencies to ensure timely, right-sited support for adults.

Qualifications

• Be a qualified Social Worker registered with Social Work England.
• Have experience working with adults in a statutory, health or social care setting.
• Understand the Care Act, Mental Capacity Act and safeguarding responsibilities.
• Confident undertaking strengths-based assessments and managing risk.
• Able to triage referrals and make proportionate, defensible decisions.
• Communicate effectively with individuals, carers, colleagues and partner agencies.
• Work collaboratively whilst using professional judgement.

Responsibilities

• Undertake strengths-based assessments, support planning and short-term casework.
• Collaborate with partner agencies to ensure appropriate support is provided.
• Triage and screen referrals within a busy front door service.
• Complete Mental Capacity Assessments, Best Interest decisions and Decision Support Tools.
• Maintain accurate and high-quality case recording in line with service standards.
