Job summary

Qualifications

• NHS experience preferred within the UK
• Experience of working in acute settings
• Willingness to work flexibly across disciplines
• Experience in discharge planning and frailty care within a hospital environment

Responsibilities

• Provide comprehensive specialist assessment and discharge planning for frailty patients
• Deliver timely and safe discharge to home or suitable environment
• Support hospital teams with admission avoidance and appropriate referrals
• Collaborate within a multi-disciplinary team including a co-located Social Worker
• Contribute to service development and 7-day RAFT coverage
