Job summary

Epsom and St Helier University Hospitals NHS Trust invites applications for a Band 6 Registered Nurse to support care homes within a proactive frailty and complex needs care model. You will lead clinical assessments, develop anticipatory care plans, and coordinate actions with MDTs to improve patient outcomes.

You will contribute to Urgent Care Plans, support discharge and community-based care, and work across care homes, primary care, and social care networks.

Qualifications

• Essential: Current Registered Adult Nurse with NMC registration.
• Continuing professional development relevant to the clinical role.
• Relevant post-registration experience in frailty, older people or long-term conditions.

Responsibilities

• Clinical assessment and intervention for frailty and complex health needs in care homes.
• Care coordination and education to care homes; leadership and practice.
• Quality improvement, audit, and data submission for service monitoring.
