Job summary

The Harbour Unit at Newton Abbot Hospital is seeking a clinical pharmacist to join its integrated frailty service for older adults. You will deliver medicines optimisation, review patients in the community and in the Harbour SDEC unit, and support polypharmacy management within a multidisciplinary team.

You will work closely with GPs, community pharmacists and hospital colleagues to improve patient safety and outcomes, using STOPP/START and other deprescribing approaches, with opportunities to

Qualifications

• Registration with GPhC.
• Postgraduate Clinical Pharmacy Diploma or equivalent clinical pharmacy experience.
• Proven Continuing Professional Development (CPD).

Responsibilities

• Lead medicines optimisation for the Harbour unit and provide a clinical pharmacy service.
• Visit patients in the community and in hospital to optimise medication and safety.
• Provide specialist clinical medication reviews and deprescribing guidance.
• Collaborate with multidisciplinary teams including GPs and care home staff.
• Support polypharmacy management and frailty care using STOPP/START.
