Job summary

Faversham Medical Practice is seeking a Frailty Nurse to lead proactive and reactive care reviews for frail patients, with focus on improving outcomes and reducing hospital admissions. You will work within a MDT, conduct advance care planning, and perform SMRs where appropriate.

The role involves care homes, patients’ homes, and clinic settings, and requires active NMC registration. You will identify eligible residents, support QOF activities, and engage in MDT meetings to coordinate patient

Qualifications

• English and Maths GCSE grade C or above or equivalent.
• Registered General Nurse with current NMC registration.
• Experience working with frail, elderly or housebound patients.

Responsibilities

• Conduct advance care planning discussions and frailty reviews in care homes, patient homes, or GP surgery.
• Carry out remote and in-practice frailty reviews with MDT support.
• Contribute to QOF-related work and chronic disease reviews.
• Maintain accurate clinical documentation and participate in audits and quality improvement.
