Job summary

SS9 South PCN is seeking a Frailty Practitioner to lead autonomous frailty assessments and urgent community-based care. The role involves home visits to frail and vulnerable patients, coordinating with GPs, INT, and social care to prevent hospital admissions and support discharge planning.

The successful candidate will have experience as an Emergency Care Practitioner, Nurse Practitioner or Nurse Prescriber with frailty expertise and must work within HCPC/NMC prescriber standards.

Qualifications

• Experienced ECP, NP or Nurse Prescriber with a specialist interest in frailty.
• Ability to perform autonomous clinical assessment, diagnosis, treatment and care planning.
• Proven ability to collaborate with GPs, INT, Community Services and social care.

Responsibilities

• Undertake urgent same-day assessments for frail patients with acute deterioration.
• Provide rapid clinical assessment and treatment to support admission avoidance.
• Assess falls, reduced mobility and suspected infections.
• Make autonomous clinical decisions and formulate treatment plans.
• Prescribe medication where qualified and authorised.
• Conduct comprehensive frailty assessments across clinical, functional, cognitive and social domains.
• Develop personalised care and support plans.
• Deliver high-quality home visiting service to housebound and vulnerable patients.
• Coordinate with GP Practices and community services to ensure timely management.
