Job summary

Westrop Medical Practice is seeking a Frailty Review Clinician to support the Enhanced Frailty Service Local Commissioned Service. The role focuses on identifying, assessing and supporting frail patients to ensure appropriate reviews and care planning.

The postholder will work autonomously within a multidisciplinary team, undertaking frailty assessments, developing care plans and coordinating referrals while aligning with local and national guidance.

Qualifications

• Advanced Clinician role—ACPs, ANPs, Paramedic Practitioners or experienced clinicians with autonomous assessment and decision-making.
• Qualified and experienced to undertake care planning for frailty patients.
• Ability to work within a multidisciplinary team and adapt to local guidance.

Responsibilities

• Undertake frailty assessments and develop personalised care plans.
• Coordinate referrals and escalation of concerns as needed.
• Identify and support patients with moderate to severe frailty to reduce hospital admissions.
• Collaborate with GPs, Nurses, Paramedics, and community services in a MDT setting.
