Job summary

Northamptonshire Healthcare NHS Foundation Trust is seeking a Community Coordinator to work with the Age Well Teams in the Northampton area. You will perform comprehensive home assessments and develop personalised care plans, coordinating referrals to ensure timely support and to help older people live independently at home.

Ideal candidates have caring experience and IT proficiency (Microsoft Office and electronic diary management).

Qualifications

• Previous experience in caring role/profession.
• IT skills including Microsoft packages and electronic diary management.

Responsibilities

• Undertake comprehensive holistic assessments in patients' own homes to create personalised care plans.
• Coordinate referrals to ensure timely support within the locality.
• Support frail older people to live independently and reduce isolation.
• Work with Befriending leads and dementia/falls supports as part of Age Well teams.
• Document care plans and monitor service delivery using clinical and organisational data systems.

Education

GCSEs minimum 5

NVQ level 3 in care field

NVQ level 4 in care field
