Job summary

Northamptonshire Healthcare Foundation Trust is recruiting a Group Facilitator/Care Coordinator to support the East Northants Frailty Team. The role coordinates and facilitates group sessions to help older adults adopt healthier lifestyle choices and achieve better outcomes.

You will work under the Team Leader (an Occupational Therapist) and GP clinical lead to undertake assessments, develop care plans and support patients to live age well.

Qualifications

• Experience in health care/community settings.
• Experience managing a caseload.
• Experience setting up groups and understanding group dynamics.

Responsibilities

• Coordinate and facilitate group sessions for patients.
• Liaise with GPs and practice teams to identify suitable individuals.
• Undertake core assessments and contribute to care plans.
• Identify opportunities to use technology to support independence.
