Job summary

Avon & Wiltshire Mental Health Partnership NHS Trust is seeking a senior practitioner to lead the FIND Team within AWP Secure Services at Fromeside Hospital. You will work closely with the Team Manager and Senior Practitioners to uphold high professional nursing standards across the service.

The post involves specialist risk assessments (HCR-20, Dundrum) and oversight of High Secure patients, with collaboration to deliver safe, effective care and support for discharged individuals up to 12

Qualifications

• Registered qualification and professional registration.
• Interest in Learning Disability and Autism, and understanding of how they present across care services.
• Experience with multidisciplinary colleagues and risk assessment/management.
