Priory Hospital Llanarth Court in Abergavenny is seeking a Registered Social Worker to join our Social Work team. The role involves working within a multidisciplinary team to deliver patient-centred care and coordinate social support.

You will assess social needs, manage risk, undertake safeguarding, and liaise with families and community networks to ensure holistic care planning. A degree in Social Work and current registration with Social Care Wales are essential.

Formación

• Degree in Social Work and current registration with Social Care Wales.
• Experience managing a caseload independently and undertaking assessments.
• Experience with risk management, safeguarding enquiries, and care planning.
• Ability to work with multi-agency partners and produce professional reports.

Responsabilidades

• Be part of a small team of Social Workers within multidisciplinary teams (psychiatry, psychology, nursing, occupational therapy) to provide a package of care.
• Identify social/non-medical needs of patients and ensure these needs are met.
• Carry out detailed risk assessment and management, safeguarding, and liaison with families and community networks.
