Cygnet Delfryn House in North Wales is seeking a dedicated Social Worker to join our new Low Secure service for women. You will provide specialist psychosocial assessment, safeguarding leadership, and recovery-focused social work to inpatients, linking with external agencies for safe discharge planning.

The service opens in early 2027 and will monitor risk in a therapeutic setting, supporting women toward more independent living with compassionate care.

Qualificações

• Qualified Social Worker with current Social Work England registration.
• Experience in mental health and inpatient settings is desirable.

Responsabilidades

• Hold and manage a ward caseload and provide therapeutic social work support.
• Assist with multi-disciplinary assessments and discharge planning.
• Refer for Care Act 2014 assessments and MHA 1983 Section 117 support.
• Engage with families, carers andTriangle of Care principles.
• Provide evidence-based, recovery-focused, person-centred social work services.
• Coordinate with external agencies to promote safe discharge and welfare.
