Job summary

Cygnet Group is seeking a confident, compassionate Social Worker to join the Forensic Inpatient team at Cygnet Delfryn House in the UK. The role focuses on psychosocial assessment, safeguarding leadership, and discharge planning for inpatients with complex needs.

The successful candidate will work within a supportive team, providing person-centered social work services, developing care plans, and coordinating with external agencies to promote safe, effective outcomes.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Experience in mental health and inpatient settings desirable.
• Knowledge of mental health legislation, safeguarding procedures, and social care frameworks.
• Experience with discharge planning and multi-agency collaboration.
• Ability to manage complex caseloads and competing priorities.
• Excellent communication and report-writing skills.

Responsibilities

• Assist in implementing and carrying out the social work department policies and procedures.
• Hold and manage a ward caseload, providing therapeutic care and support.
• Assist with multi-disciplinary assessments and discharge planning.
• Refer for Care Act assessments and support under the Mental Health Act 1983.
• Support families with assessments and guidance; apply Triangle of Care principles.
• Attend statutory and mandatory training; provide evidence-based, recovery-focused social work.
• Collaborate with Service Users on care planning and support tools.
• Liaise with professionals and agencies to ensure integrated service delivery.
