Job summary

Cygnet Elowen Hospital in Derbyshire seeks a confident, compassionate full-time Social Worker to join a dedicated inpatient team. The role focuses on specialist psychosocial assessment, safeguarding leadership, and discharge planning for service users in two wards: Nova Ward and Alina Ward.

You will work within a supportive, multidisciplinary environment to promote recovery, independence and safe care pathways at this specialist site.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Experience in mental health and inpatient settings desirable.

Responsibilities

• Assist in implementing Social Work department policies and procedures.
• Manage a ward caseload and provide therapeutic care and support.
• Assist with multidisciplinary assessments and discharge planning.
• Support Care Act 2014 assessments and S117 Mental Health Act responsibilities.
• Engage families/carers with assessments and Triangle of Care principles.
• Provide evidence-based, recovery-focused, person-centred social work.
• Collaborate with professionals to achieve integrated service delivery.
