Job summary

Cygnet Health Care Limited is recruiting a confident and compassionate Social Worker to join the new Forensic Inpatient unit at Delfryn House in North Wales. This full-time role focuses on specialist psychosocial assessment, safeguarding leadership and discharge planning for women in care.

As part of a supportive multi-disciplinary team, you will deliver person-centred social work, contribute to risk management and act as a key link with external agencies to promote recovery and independence.

Qualifications

• Qualified Social Worker with current Social Work England professional registration.
• Experience working within mental health and inpatient settings is desirable.
• Knowledge of mental health legislation, safeguarding procedures, and social care frameworks.
• Experience with discharge planning and multi-agency collaboration.
• Ability to manage complex caseloads and competing priorities.
• Excellent communication and report-writing skills.

Responsibilities

• Assist in implementing the Company's policies and procedures within the Social Work department.
• Hold and manage a ward caseload, ensuring therapeutic care and support per social care needs.
• Assist with multi-disciplinary assessments of service users through referral to discharge.
• Support assessments under the Care Act 2014 and Section 117 of the Mental Health Act 1983.
• Support families/carers with assessments and information; apply Triangle of Care principles.
• Attend statutory and mandatory training; provide a recovery-focused, person-centred service.
• Assess and collaborate with service users in planning and implementing care plans and tools.
• Liaise with other professionals to ensure integrated service delivery and optimal functioning.
