Job summary

Cygnet Delfryn House, a new Low Secure service for women in North Wales, is seeking a confident, compassionate full-time Social Worker to join our team. The role focuses on specialist psychosocial assessment, safeguarding leadership and discharge planning within an inpatient setting.

The post holder will liaise with external agencies to promote safe discharge and ensure compassionate, recovery‑oriented care for service users. Prior mental health/inpatient experience is desirable.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Experience in mental health and inpatient settings is desirable.
• Knowledge of mental health legislation, safeguarding procedures and social care frameworks.
• Experience with discharge planning and multi‑agency collaboration.
• Ability to manage complex caseloads and competing priorities.
• Excellent communication and report‑writing skills.

Responsibilities

• Assist in implementing and carrying out the Social Work department policies and procedures.
• Hold and manage a ward caseload, ensuring therapeutic care and support.
• Assist with multi‑disciplinary assessments and discharge planning.
• Refer for Care Act 2014 assessments and Section 117 Mental Health Act support.
• Support families using assessments and Triangle of Care principles.
• Attend statutory and mandatory training as required.
• Provide an evidence‑based, recovery‑focused, person‑centred Social Work service.
• Plan and implement care plans and assessment tools with Service Users.
• Assist service users with home risk assessments, group work, and discharge planning with community professionals.
• Liaise with other professionals to ensure integrated service delivery.
