Job summary

Change Grow Live Nottinghamshire is seeking an enthusiastic Floating Support Recovery Worker to help clients in housing crisis. You will work as part of the floating support housing team, supporting vulnerable individuals who may experience cuckooing, unstable housing, learning disabilities and brain injuries.

Qualifications

• An understanding of the harmful effects associated with drug and alcohol use on health, social welfare, housing, employment and personal relationships.
• Minimum of 12 months experience facilitating tenancy sustainment and building positive community connections.
• Experience engaging with a wide variety of individuals from different backgrounds and needs.
• Ability to work with and manage challenging behaviour.

Responsibilities

• Establish effective communication and referral pathways for individuals at risk of homelessness.
• Provide wrap around support in emergency/temporary/supported accommodation across the area.
• Prioritise caseload based on crisis intervention and needs severity.
• Attend multi-disciplinary team meetings linking health and social care, peer support networks and community assets.
• Collaborate with local authority, health & social care partners to prevent homelessness and improve joint working.
• Enhance partnership work with primary care, mental health services and housing organisations to support tenancy stability.
