Job summary

Change Grow Live in Hastings is seeking a Housing Support Recovery Worker to assess and engage service users, coordinating with housing and health services to promote tenancy sustainment and access to treatment pathways.

You will co-locate between substance misuse settings and housing offices, deliver psycho-social interventions, and work within a multidisciplinary team to support individuals at risk of homelessness.

Qualifications

• Minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections.
• Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.
• The ability to work with and manage challenging behaviour.
• Have a valid UK driving licence, and use of own car, to work flexibly across different sites as required.

Responsibilities

• Establish effective communication and referral pathways for individuals who are at risk of homelessness and in contact with other health and care agencies.
• Provide wrap around support in emergency/temporary/supported accommodations across the area.
• Prioritise caseload depending on crisis intervention and severity of needs.
• Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
• Work closely with the local authority, Health & Social care sector, partners and any provision linked to preventing Homelessness and improve joint working.
• Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting individuals at risk of losing tenancies.
• Provide wrap around support on budgeting, education, training and employment whilst continuing to support their engagement in structured services.
