Responsibilities

• Develop personalized support plans with clients.
• Support clients to develop life skills such as budgeting and basic household tasks.
• Provide tenancy sustainment advice and explain tenancy agreements.
• Offer emotional support and crisis intervention as needed.
• Help clients access local services like healthcare, employment, education and benefits.
• Respond to emergencies and escalate concerns to ensure safety.
• Identify and report safeguarding concerns in line with policy and guidance.
• Coordinate with social services, housing providers and other agencies.
• Conduct regular checks of living environments for safety and standards.
• Maintain accurate records of contacts, interventions, and progress.
• Advocate for clients to uphold their rights and preferences.

Education
