Job summary

Plus Dane in Cheshire is seeking a Floating Support Advisor to deliver high-quality, person-centred support to vulnerable people and sustain tenancies across the Cheshire region. You will assess needs, develop tailored plans, collaborate with partner agencies, and help customers build independence.

Qualifications

• Experience in supported housing or similar support-based environment.
• Ability to work in partnership with external agencies.
• Strong organisational skills and ability to manage priorities.
• Accurate records and clear written documentation.
• Proficient IT skills and systems confidence.
• Customer-focused approach with strong communication.
• Full UK driving licence and access to a vehicle.

Responsibilities

• Manage referrals, complete assessments, and sign up new clients.
• Develop and deliver individual support plans for customers.
• Help customers build independence and reduce service reliance.
• Collaborate with external agencies to improve outcomes and prevent homelessness.
• Review support plans to meet customer and stakeholder expectations.
• Maintain accurate case records and documentation.
• Contribute to service development and raise the profile of the service.
• Act as a main contact for colleagues regarding external services.
