Aufgaben

• Manage a caseload of registered and temporary patients with a wide range of health needs
• Hold face-to-face and telephone consultations and home visits in line with the practice timetable
• Make autonomous decisions on presenting problems, including self-referrals and referrals from other staff
• Assess undifferentiated and undiagnosed problems and screen for disease risk factors and early illness
• Develop care plans with patients in line with current disease management protocols
• Admit and discharge patients to and from the caseload and refer to other care providers
• Compile and issue computer-generated acute and repeat prescriptions, prescribing from the formulary where clinically appropriate
• Provide counselling and health education
• Record clear, timely consultation notes and deal with paperwork and correspondence
• Carry out a weekly out-of-hospital ward round in community settings with multidisciplinary teams
• Contribute to audit, clinical standard setting and lifelong learning
• Maintain computer-based patient records, including summarising and read-coding, and keep patient information confidential
