Qualifications

• Qualified Social Worker registered with Social Work England.
• Experience in assessment, risk management and safeguarding.

Responsibilities

• Conduct Care Act assessments, safeguarding and Best Interests decisions.
• Engage in multi-agency working and hospital discharge planning.
• Collaborate with adults with cognitive and physical disabilities to promote wellbeing.
• Support long-term and complex cases through C&P D teams.
