Job summary

Caritas Recruitment Limited is seeking a dedicated Social Worker to join our Disabilities/LD team in Wolverhampton. You will assess, plan, advocate, and coordinate care to empower clients with disabilities and learning difficulties.

The ideal candidate holds a recognised SW qualification and is registered with Social Work England, with strong communication and problem-solving skills to support clients and families effectively.

Qualifications

Recognised social work qualification (BA/MA).
Registered with Social Work England.
Experience with disabilities and learning difficulties.
Strong communication and interpersonal skills.
Commitment to person-centred practice.

Responsibilities

Conduct comprehensive assessments to identify needs and strengths.
Develop and implement personalized care plans with clients and families.
Provide ongoing support and advocacy to access services.
Collaborate with multidisciplinary teams for holistic care.
Monitor and review care plans to ensure relevance and effectiveness.
Document casework and maintain records per policy.
