Job summary

Caremark Pulborough and Horsham seeks a Team Leader to deliver high-quality, person-centred care. You will conduct care reviews, needs assessments, and risk assessments, creating tailored plans that fit each individual’s preferences and support requirements.

Field-based leadership includes mentoring Care Assistants, observing care delivery, and ensuring safety, compliance with CQC standards, and responsive on-call support for emergencies and urgent visits.

Qualifications

• At least 2 years’ experience in a care setting (home care preferred)
• Minimum of Level 3 Health & Social Care qualification
• A full UK driving licence
• Participate in the out-of-hours on-call rota to respond to emergencies and ensure service continuity
• Strong written and verbal communication skills
• Compassionate and calm under pressure
• Knowledge of CQC standards and regulatory compliance
• Knowledge of safeguarding protocols and incident reporting procedures.

Responsibilities

• Lead frontline care reviews, needs assessments and risk assessments to create tailored care plans.
• Observe care delivery in the field, mentor Care Assistants and ensure services are safe and compliant.
• Coordinate with office staff and monitor service quality to meet regulatory expectations (CQC).
• Respond to emergencies or urgent situations and provide on-call support as needed.
