Caremark Merton is seeking a Field Care Supervisor to oversee field operations and ensure high-quality support for clients and workers in a defined area.

You will conduct assessments, coordinate staffing, and mentor care workers while maintaining thorough records and reporting to Care Management. This role is essential for delivering reliable community care and improving service quality.

Qualifications

• Experience supervising Care & Support Workers in field operations.

Responsibilities

• Carry out field-based quality control and support Care & Support Workers within a defined area.
• Conduct Individual Needs Assessments for new and potential clients.
• Carry out risk assessments as part of the Individual Needs Assessment and update these as required as part of the individual’s risk management.
• Ensure all client visits are covered and be on-call for emergencies.
• Liaise with Care Coordinators regarding recruitment, permanent allocations and Care & Support Agreements.
• Identify suitable Care & Support Workers to cover for absences and arrange replacements.
• Facilitate induction and in-field training for new Care & Support Workers.
• Monitor and mentor all Care & Support Workers in the field.
• Encourage ongoing professional development and maintain documentation in clients’ homes.
