Job summary

Lloyd's Register International invites applications for an Outreach Practitioner to work with people who use substances in the Sandwell area. The DEMO project supports those with complex health concerns, engaging them in harm reduction approaches and linking to wider services.

You will join a team led by the DEMO Advanced Nurse Practitioner, handle near-fatal overdose response coordination, and use Microsoft tools and case management systems to document care.

Qualifications

• Excellent engagement, communication, listening and organisational skills.
• Ability to adapt to changing needs of a vulnerable target group.
• Ability to work with Microsoft programs and case management systems.
• Experience in harm reduction or related health/social care settings is desirable.

Responsibilities

• Engage with people who use substances in the Sandwell area.
• Coordinate responses to near-fatal overdoses to reduce risk and harm.
• Deliver harm reduction interventions and support engagement with health services.
