Hope House Children’s Hospices has an opportunity for a Family Support Worker to join our team to work across Wrexham and Flintshire. You will join us on a full-time, permanent basis. In return, our Family Support Worker will receive a competitive salary of £28,118 to £29,929 per annum.
Hope House Children’s Hospice is a centre of excellence for children’s palliative care, ensuring every family facing the death of a child gets the very best care and support when and where they need it. No one should face the death of a child alone.
We are recruiting for a Family Support Worker to work in the community across Wrexham and Flintshire. Our Family Support Worker will work closely with our Community Nurse and play a key role in supporting the comprehensive pre and post bereavement service for each identified child and their family, including anticipatory grief support, end of life planning, and advocacy. You will work alongside the wider teams at Hope House and Tŷ Gobaith, and with external care providers within the local communities and professional networks in Wrexham and Flintshire.
You will need to be a compassionate, caring, and organized individual who can demonstrate a versatile approach to work, ensuring the holistic palliative care needs of the family are met. Excellent communication skills and the ability to collaborate with families and professionals are essential.
