REPORTS TO: HFM Program Supervisor
HOURS: Part-time (20 hours per week)
SALARY: $19.27-$21.20 per hour
JOB SUMMARY:
Are you passionate about supporting families during an important time in their lives- caring for a
newborn or welcoming a new child? SPCC is seeking a compassionate, dedicated professional to join
our Healthy Families Monroe team. In this dual-role position, you will provide strengths-based, homevisiting services to families, offering parenting support, case coordination, advocacy, and connections
to essential resources. You’ll make a lasting impact by building relationships that foster safe, nurturing
environments for infants and young children.
RESPONSIBILITIES INCLUDE:
• Maintain a caseload of 8–12 families, delivering consistent, home-based support
• Provide case coordination, emotional support, parenting education, and resource referrals
• Accurately document services and maintain up-to-date records in the HFNY data system
• Participate in regular supervision and training to support reflective, trauma-informed practice
• Build strong relationships with families and community agencies to ensure quality care
• Occasionally assist with assessments for newly referred families
QUALIFICATIONS:
• High school diploma required; some college coursework preferred
• 3–5 years’ experience in human services, especially with children and families
• Knowledge of infant development and parent-child attachment
• Strong communication skills and cultural humility
• Ability to handle crisis situations with a calm, grounded presence
• Lived experience similar to the families we serve is a plus
