Job summary

Bradford Children and Families Trust is seeking a dedicated Family Support Worker in Bradford to lead holistic assessments and develop personalised plans for children and families. You will support participation by young people in assessments and reviews while managing a caseload and coordinating multi-agency interventions.

Regular reviews with families and partners are a core part of the role. The position requires safeguarding knowledge, GCSEs, IT literacy, and Level 3 Health and Social Care

Qualifications

• Recent direct experience with children, young people and families.
• Level 3 Health and Social Care qualification or equivalent.
• GCSE English and Maths or equivalent.
• Understanding of safeguarding and promoting welfare of children and young people.

Responsibilities

• Undertake holistic and strength-based assessments and devise plans for babies, children, young people and families.
• Ensure participation of children and young people in their assessment, plans and reviews.
• Manage caseload and coordinate multi-agency support to deliver tailored family interventions and track progress over time.
• Arrange and chair reviews to monitor progress with children, young people and partner agencies.
