Job summary

Community Care Live is seeking a Family Practitioner to support children and families toward positive change. You’ll manage a varied caseload, deliver tailored interventions, and work with Social Workers and other professionals to inform assessments and family plans.

Using the Hampshire Approach, you’ll provide practical and emotional support, build relationships, and help families access services. You’ll prepare reports and maintain records as part of a collaborative team.

Qualifications

  • Experience working with children, parents or carers.
  • Strong verbal and written communication skills.
  • Mediation and ability to manage challenging situations.
  • Ability to produce clear reports and use IT systems confidently.
  • Flexible, adaptable and committed to inclusive practice.

Job description

As a Family Practitioner, you’ll play a vital role in supporting children and families to achieve positive and lasting change. You’ll manage a varied caseload, delivering tailored interventions that respond to each family’s unique circumstances. Working closely with Social Workers and other professionals, you’ll contribute to assessments and help develop family plans that put children’s needs at the heart of decision‑making.

You’ll join a dedicated team using the strengths‑based Hampshire Approach to empower families and keep more children safely at home. Through practical support, relationship‑building, and solution‑focused interventions, you’ll help families build on their strengths and overcome challenges, making a real difference to their lives.

What you’ll do:
  • Work directly with children, parents, and carers to provide practical and emotional support.

  • Undertake assessments and Children in Need visits alongside Social Workers.
