Job summary

The Buckinghamshire Council seeks a Family Hub Support Worker to join the Family Hub Service. You will work with families in hubs, homes and community settings, delivering universal and targeted interventions to improve outcomes for children with additional needs.

Based across Buckinghamshire with flexible diary management, you will engage with partners and signpost services while promoting inclusive practice and supporting transitions between universal and targeted services.

Qualifications

• Experience working with children, families and carers within multi-agency environments.
• Knowledge of inclusive early years practice, child development and SEND pathways.
• Ability to build trusting relationships and communicate with families and professionals.
• Experience delivering targeted interventions and promoting inclusive practice.

Responsibilities

• Deliver universal and targeted family interventions in hubs, homes and community settings.
• Identify emerging needs early and signpost to appropriate services.
• Support children with additional needs to achieve good developmental outcomes.
• Collaborate with early years settings, SEND professionals and health services to coordinate support.
