Job summary

Hastings & Rother Healthcare in Hastings, UK, seeks an Extended Scope Practitioner (nurse or paramedic) for the Enhanced Care Team to deliver patient-centered Frailty and Palliative care. You will perform home visits, clinics and telephone consultations, ensuring high-quality, rapid assessment and care planning.

Qualifications

• Dynamic, enthusiastic Extended Scope Practitioner (nurse or paramedic) in Frailty/Palliative Care.
• Experience in holistic patient assessment and safe clinical decision-making.

Responsibilities

• Conduct home visits, face-to-face clinics and telephone consultations for frailty, palliative care, and related conditions.
• Triage needs, coordinate investigations and referrals, and manage chronic disease.
• Participate in holistic care planning including care homes, dementia and safeguarding, with a multidisciplinary team.
