Zusammenfassung

Hastings & Rother Healthcare is seeking a full-time Extended Scope Practitioner to join the Enhanced Care Team, covering frailty, palliative care, care homes and dementia. The work centres on home visits, clinics and telephone triage, with independent assessment, diagnosis, prescribing and end-of-life care coordination.

You will lead ward rounds in care homes, support over-75s triage and contribute to a wider multidisciplinary primary care team.

Qualifikationen

• Registration as an NMC (UK) or HCPC professional, with annual renewal.
• Post-graduate study including a Level 6 or 7 mentorship module.
• Experience as an extended-role clinician in a primary care setting.

Aufgaben

• Lead home visits, face-to-face clinics and telephone consultations for patients with medical and social needs.
• Assess, diagnose and manage patients independently from history-taking to evaluation of care.
• Triage patients (including over-75s) and refer for investigations and specialist input as appropriate.
• Prescribe medicines and carry out treatments independently, within scope of practice.
• Coordinate holistic assessment and end-of-life care for palliative patients.
• Provide care for frail and elderly patients in care and nursing homes, including weekly ward rounds.
• Manage dementia, learning disability and long-term conditions, including complex comorbidities and QOF reviews.
• Manage memory loss referrals to MAS and related investigations and treatment.
• Complete, review and audit RESPECT forms with patients and carers.
• Coordinate care with District Nurse team, hospice, social care and SOAHMS.
• Keep accurate clinical records and participate in audit, quality improvement and safeguarding.
• Complete training in Level 7 physical assessment, a Level 7 frailty module and non-medical prescribing.
