You will undertake home visits, clinics, triage, and ward rounds, review RESPECT forms, manage memory loss patients, and coordinate investigations, ensuring safe, evidence-based practice and thorough documentation.

Qualifications

• Leadership in clinical care for frail/palliative patients.
• Autonomous assessment and decision-making within scope.
• Non-Medical Prescribing and memory loss management.
• Home visits, clinics, triage and multi-disciplinary coordination.

Responsibilities

• Lead care for frail and palliative patients with advanced clinical reasoning.
• Undertake home visits, face-to-face clinics and triage activities.
• Review RESPECT forms with patients and carers; coordinate memory loss assessments.
• Prescribe within scope and arrange necessary investigations or input.
• Maintain documentation, governance, and continuing professional development.
