Experienced Social Worker - Adult Social Care Team

Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for an Experienced Social Worker within the Adult Social Care Team, based in Swindon.

The salary for this permanent Experienced Social Worker role is up to £45,495 per annum.

Responsibilities include:

1. Undertaking complex person-centred assessments, including mental capacity assessments, to support individuals in achieving their outcomes and promoting independence.
2. Supporting carers in complex situations.
3. Accurately recording all activity in the social care records database, adhering to work targets.
4. Gathering sufficient information to assess individuals' needs against eligibility criteria.
5. Contributing knowledge about available services and signposting or advocating for individuals.
6. Applying advanced assessment, support planning, and risk management skills in line with council policies.
7. Supervising team members monthly, ensuring timely and recorded supervision, and addressing practice concerns.
8. Representing the council in multidisciplinary collaborations and supporting decision-making processes.
9. Preparing reports and presenting evidence in court, including Court of Protection and High Court.
10. Following council policies and procedures at all times.
11. Proactively contributing to team development and service improvement.
12. Leading peer development sessions within and across teams.
13. Providing statutory support to adults with care needs and their families, including assessments, support planning, and safeguarding.

Requirements:

• A recognised Social Work qualification or equivalent.
• Understanding of relevant legislation.
• Registered or eligible for registration.
