Are you ready to embark on a fulfilling journey in the world of social work? We're seeking an experienced Social Worker to join our Hospital Discharge Team, focusing on innovative front-door hospital approaches for swift discharge pathways.
Positioned in three acute hospitals, our teams provide crucial advice, assessments, and discharge planning, ensuring safe and timely transfers of care through a collaborative, multi-disciplinary approach.You will also undertake assessments of our clients in their own homes post-discharge.
As a key team member, you'll conduct complex assessments, serve as an Investigating Officer in safeguarding cases, and apply expertise in the Mental Capacity Act 2005 and the Care Act 2014.
Bring at least 2 years of post-qualifying experience and Social Work England registration, confidently offering peer supervision and support to fellow social workers and customer coordinators. Your commitment, coupled with first-class knowledge of relevant legislation, will drive the positive change we aim to achieve.

Specific duties and responsibilities include:
To undertake person-centred assessments (including Mental Capacity Assessments) withcustomers and to plan for the provision of support and/ or services with customers in order toassist them meet their outcomes and to live as independently as possible, fulfilling theirindividual potential. To ensure accurate and timely recording of all information/assessmentactivity onto the Social Care records database in accordance with work targets set by theHospital Team Manager and Supervisor.
To gather sufficient information about customers in order to be able to accurately assess theirneeds against eligibility criteria. To ensure that local knowledge about universally availableservices is up to date including what is available within the voluntary and community sector, aswell as the Departments commissioned and contracted services and to signpost/advocate forcustomers accordingly.
To have a well-developed understanding of assessment and the management of risks forindividuals and to be able to apply Council policy and support customers to manage riskappropriately. To identify where vulnerable adults may be at risk and to act in accordance withthe Departments Safeguarding policies and procedures. To be able to undertake theInvestigating Officer role in accordance with Departments Safeguarding policies andprocedures.
To participate in Specialist Response/Duty Rota enabling access to services to work effectively,making decisions about risk, eligibility and priority.
To work as part of a multi-disciplinary team, with health and social care colleagues to ensureeffective communication/information sharing and to support assessment/decision makingprocess.
To ensure that Council corporate and departmental Policies and Procedures are followed at alltimes.
To contribute proactively to the effective working of the team with positive attitude, by sharingknowledge, offering advice and support and by preparedness to be involved in the developmentof services.
To fully engage and prepare for own supervision and appraisal through self-evaluation, ensuringthat continual professional development is evidenced.
To be an ambassador for the Council and the department at all times, always representing theCouncil positively, professionally and appropriately at meetings with customers, externalpartners and agencies.
