Job summary

Voluntary Action Doncaster is seeking an Out-of-hours Support Worker to assist older people being discharged from hospital and to prevent unnecessary readmission. The role involves Thursday evening shifts with additional hours across the year, including potential day-time bank hours when available.

Training will be provided to enable a practical, supportive service within the community. The post focuses on helping individuals return home safely, regain independence, and access practical help

Responsibilities

• Work closely with health and social care staff to coordinate discharge support.
• Complete referrals and allocate support where needed.
• Deliver flexible, person-centred assistance to prevent hospital readmission.
• Provide telephone support to service users and families as required.
• Maintain accurate records and prepare written reports.
