Job summary

Touchstone, part of the Leeds Mental Wellbeing Service (LMWS), is hiring a Locality Primary Care Mental Health Support Worker in Leeds. You will deliver short-term one-to-one support, health equity work, and co-facilitate wellbeing groups within LMWS across community hubs.

As part of a multi-agency team, you will help increase inclusivity, signpost to the right services, and support people to access mental health interventions, including outreach and volunteer collaboration.

Responsibilities

• Inclusive engagement: understand mental health inequalities in Leeds and how some communities are at higher risk of poorer mental health; deliver active in-reach into communities and signpost to the right support.
• Service delivery: plan and facilitate interventions alongside colleagues; support access to mental health interventions; co-deliver group interventions for specific communities or universal groups.
• General: feed information to management structures; maintain professional boundaries; represent Touchstone; manage risk and ensure accurate records and monitoring; undertake duties as directed by management.
