Responsibilities

• Provide person-centred support to individuals with a range of needs.
• Promote independence, dignity, choice and control in daily life.
• Support with daily living activities and personal care where required.
• Assist with meal preparation, nutrition and hydration.
• Support individuals with Learning Disabilities, Autism, Mental Health needs or complex needs.
• Encourage participation in social, recreational and community activities.
• Assist individuals to attend appointments and access community services.
• Follow individual support plans, risk assessments and safeguarding procedures.
• Provide emotional support and promote wellbeing.
• Support development of life skills and independence.
• Communicate with individuals, colleagues and professionals.
• Monitor changes in health, behaviour or wellbeing.
• Maintain a safe, clean and supportive environment.
• Work effectively as part of a wider support team.
