Job summary

Gateshead Council’s Adult Social Care is recruiting a Supported Living Service Support Worker to help adults with care and support needs live independently and safely in their own homes and communities. You will build trusting relationships, support tenancy maintenance, and enable people to pursue meaningful life choices.

You will work within person-centred plans, follow Care Quality Commission standards, and collaborate with colleagues and external professionals.

Qualifications

• Level 2 Health and Social Care or equivalent.
• Understanding of adult social care legislation and safeguarding.
• Understanding of Supported Living values: choice, dignity, independence, inclusion.
• Experience with adults with learning disabilities or complex needs or transferable care skills.
• Strong communication with individuals, families, colleagues and professionals (written, verbal, IT).
• Strong organisation and time-management skills; ability to work independently within a team.

Responsibilities

• Promote rights and dignity of tenants considering individual needs and outcomes.
• Provide personal care support where required.
• Promote independence, self-management and citizenship under the Enablement Framework.
• Support tenants in community settings according to their care plans.
• Assist tenants to engage in activities at home and in the community.
• Liaise with colleagues, tenants, advocacy services, carers, and professionals to coordinate care.
• Carry out household administration and maintain records; monitor and evaluate support plans.
• Assist with household tasks, meal planning/preparation and general living skills; ensure safety and security.
