Job summary

Praxis Care is seeking a dedicated Support Worker for our Community Learning Disability Service in Portadown. You will be part of a supportive team delivering person-centred care to individuals, promoting independence and dignity.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Desirable: experience in health and social care.

Responsibilities

• Support individuals to develop daily living and personal care skills.
• Collaborate with team leaders to deliver individual and group support plans.
• Maintain a safe environment and manage challenging situations with appropriate language.
• Ensure medication is stored and administered per policy where required.
• Complete administrative tasks and follow service reporting guidelines.
• Follow all policies and participate in required training.
