Job summary

Connect2Swindon is seeking compassionate Reablement Support Workers to assist individuals in regaining independence at home and in the community. You will provide practical and emotional support, help with daily living activities, and ensure safe transport to appointments and activities.

You will collaborate with healthcare professionals to deliver person-centred care plans, maintain accurate records, and uphold safeguarding and equality standards across all interactions.

Qualifications

• Previous experience as a Care Support Worker or similar preferred.
• Valid driving licence with a clean record.
• Experience driving clients safely, including those with mobility or other needs.
• Understanding of agency compliance standards and care regulations.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to equality, diversity and inclusion in the workplace and care delivery.
• Relevant certification or willingness to undertake training in reablement or social care.
• Basic literacy and numeracy skills required.
• DBS check and other statutory clearances as applicable.

Responsibilities

• Engage with clients to support their daily routines and personal goals.
• Safely drive clients to appointments, social events, and community activities.
• Conduct regular vehicle safety and maintenance checks.
• Attend team meetings and training sessions to enhance skills and knowledge.
• Complete documentation accurately and promptly using digital or paper systems.
• Build trusting relationships with clients, respecting their choices and cultural backgrounds.
• Adapt support strategies to meet the evolving needs of each individual.
• Respond promptly and appropriately to any incidents of unauthorised activity, ensuring client safety.
• Collaborate with colleagues and external agencies to provide holistic support.
