Job summary

mbi Medical Ltd seeks a practitioner to provide assessment, support planning and reviews for vulnerable people, ensuring risk is managed and person-centred outcomes are identified.

You will work with health, early help, community and voluntary sectors, align with NHS and social care legislation including the Mental Capacity Act, and support carers while facilitating access to welfare benefits advice and direct payments where appropriate.

Qualifications

• Professional assessment to support risk management and outcome identification.
• Knowledge of NHS and social care legislation and Mental Capacity Act.
• Ability to signpost to specialist welfare benefits services where appropriate.

Responsibilities

• Work closely with colleagues in health, early help, community and voluntary sectors to meet person centred outcomes.
• Work within organisational policy and legislative requirements.
• Undertake professional assessment to assist customers to manage risk and identify person centred outcomes.
• Deliver the service in accordance with NHS and social care legislation and eligibility criteria.
• Undertake best interest assessments taking account of the Mental Capacity Act.
• Support carers in their role.
• Ensure support plan reviews are carried out timely and person centred.
• Provide information and advice to help people stay healthy, plan for their future needs, use services effectively, take responsibility for their own health and care, and maximise prevention and recovery services.
• Encourage and support people to use personal budgets and direct payments where possible.
• Undertake straightforward financial assessments and provide welfare benefits advice signposting to specialist services where appropriate.
