Job summary

FT Care – Domiciliary Care Service is seeking a Community Support Worker to empower individuals with diverse needs to live as independently as possible within the local community. You will provide person-centred care, promote dignity, choice and active participation, and collaborate with families and professionals to adapt care plans.

The role focuses on personal care, community integration, and life skills development, with an emphasis on safeguarding, advocacy and emotional wellbeing.

Qualifications

• Experience in social care or healthcare is advantageous; full training provided.
• Outstanding communication, patience and active listening required; problem-solving abilities important.
• Values: equality, diversity, inclusion; committed to compassionate care.
• Valid UK driving licence and access to a reliable vehicle required.
• Satisfactory DBS clearance is expected.

Responsibilities

• Personal Care: assist with daily living activities including hygiene, dressing, meals, and medication administration per care plans.
• Community Integration: accompany clients to social activities, appointments, classes, and events to build confidence and local connections.
• Skill Development: support budgeting, cooking, transport use, and household management to foster independence.
• Emotional Wellbeing: provide empathetic support and encourage mental health and resilience.
• Advocacy & Safeguarding: advocate for clients’ rights and report concerns per safeguarding policies.
• Collaboration: work with family, social workers, healthcare professionals and multidisciplinary teams to adapt care plans.
• Record Keeping: maintain daily logs, incident reports and care plan evaluations per regulatory standards.
