Job summary

You Matter is seeking a Care & Support Practitioner to join our team at Hestia House, Liverpool. You will provide compassionate, person-centred care and work to empower residents experiencing mental health challenges to make positive changes in their lives.

You will build relationships with residents, support their independence and delivery of care plans, and coordinate with partner agencies to ensure safeguarding and rights are respected.

Qualifications

• Experience in care or support services.
• Experience providing personal care to adults.
• Knowledge of safeguarding protocols and policy.
• Ability to document accurately and concisely.
• Understanding of rights and statutory services.
• Experience with risk assessments and care plans.

Responsibilities

• Introduce and build relationships with new residents and induct into the service.
• Ensure residents understand rights and responsibilities and engage with services.
• Support independence and work toward goals defined in care plans.
• Update risk assessments and care plans; manage documentation and records.
• Participate in evening group work and activities with residents.
• Provide information and guidance in line with care plans and policies.
