Job summary

You Matter is recruiting a Care & Support Practitioner to work at Hestia House. You will provide empathetic support to residents and help create a safe, dignified environment while delivering person-centred care in line with care plans.

The role involves building relationships with residents, supporting daily living, documenting care activities, and participating in group activities. Full training and a supportive team are provided; the salary is £30,000 per annum.

Qualifications

  • Qualification or working experience in Care or Support Services.
  • Experience providing personal care to vulnerable adults.
  • Knowledge of Health & Safety issues.
  • Understanding of mental health, homelessness, substance misuse and offending.
  • Knowledge of Adult safeguarding protocols.

Responsibilities

  • Build relationships with residents and induct them into the service.
  • Explain residents' rights and responsibilities during their stay.
  • Support residents to maintain independence and achieve goals.
  • Update risk assessments and communicate care plans as required, including medication management.
  • Reference care and support plans for person-centred needs and record information.
  • Deliver evening group work and activities; participate in resident meetings.
  • Care duties and safeguarding prompts; ensure dignity and respect in all interactions.
  • Maintain up-to-date records and day-to-day paperwork in resident files.
