Job summary

AWD online is seeking an Early Years Inclusion Practitioner to support children aged 0-5 with emerging needs within a local Family Hub. You will build relationships with families, use assessments, and collaborate with health, SEND teams and community partners to promote inclusive practice.

The role involves identifying needs, providing targeted support, and helping families navigate services. It is a full-time equivalent post working across the county with Office-based duties.

Qualifications

• Experience of working with children 0-5 with complex needs.
• Strong communication with parents and multi-agency partners.
• Good IT skills and ability to maintain records.

Responsibilities

• Identify emerging needs through observation and assessment.
• Provide universal and targeted support for children and families.
• Work with parents to implement practical strategies and review progress.
