Job summary

AWD online is seeking an Early Years SEND Practitioner to help children aged 0–5 and their families access timely support through Family Hubs and community networks. The role focuses on early identification, assessment and linking families to services, with a drive to promote inclusion and positive outcomes.

You will work across Family Hubs, health professionals and other providers, implementing evidence-based strategies and maintaining clear records.

Qualifications

• Experience working with children aged 0–5 with complex needs or disabilities.
• Experience working with parents on joint assessment, review and teaching strategies.
• Excellent written and oral communication with diverse audiences.
• Ability to use multiple assessment techniques and plan targeted support.
• Ability to set targets and review progress for individual children.
• Competent IT skills; routine record-keeping and reporting.
• Knowledge of early child development and EYFS framework.
• Understanding of SEND Code of Practice and service pathways.
• Strong organisational and interpersonal skills; effective diary management.
• Ability to work both in a team and independently; driving across the county.

Responsibilities

• Identify emerging needs by observing children and using evidence-based assessment tools.
• Deliver universal and targeted early interventions and monitor progress.
• Support families to plan and apply strategies.
• Help families access inclusion pathways, financial support and community resources.
• Collaborate with health services, early years providers and SEND teams.
• Deliver drop-ins, short-term workshops and family sessions.
• Maintain accurate records and generate reports per service protocols.
• Attend multi-agency reviews and share relevant information.
• Promote inclusion across Family Hubs and wider networks.
